No certificate is required for the first seven days of any absence due to illness.
If you are absent for more than four days due to illness, your employer may ask you to complete a self certificate form SC2; they can supply this to you or you can download from https://www.hmrc.gov.uk/forms/sc2.pdf
Patients can self certify for seven days before requesting a sick certificate (Med3) as per government guidelines. Please see the updated government guidelines using this link: https://www.gov.uk/taking-sick-leave
MED 3 Certificates
If your illness results in absence of seven or more days, you should arrange an appointment with a Doctor and request a sickness certificate. These are free of charge. The Practice Nurses cannot issue sickness certificates.
Please note: Requests for sick notes to be issued prior to seven days of self certification are chargeable at a fee of £20.00
