How to register

We have an open list and welcome requests for registration from patients living in  in the practice area. We accept patients from the following areas: Broom, Campton, Chicksands, Clifton, Clophill, Gravenhurst, Haynes, Henlow, Ireland, Langford, Meppershall, Old Warden, Shefford, Shillington, Stanford and the Stondons.

From April 2015 all GP practices are required to provide all their patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them. 

Every patient at this practice has been assigned to one of the GP partners.

Please be assured that this will not impact your experience of the care the Practice offers, the provision of appointments, your treatment, or prevent you from seeing any GP in the practice as you have always done.

It is important to be clear that you do not need to wait and only see your named GP, you will be informed who your named GP is at the time of registration.

You can register by completing  forms that are available from reception. We will make a note on your records if you prefer to see a particular doctor or nurse.

We recommend that all new patients undertake a health check with a Practice Nurse or Health Care Assistant, however if you are taking regular medication you must have a new patient check with a Nurse or Doctor as we will be unable to issue you with any repeat medication until you have been seen.

You may print off and complete the following two forms to register with the Practice however they must be brought into the Practice for completion.

Please complete both the following forms and include your NHS number, if you are a new Patient to the Practice you may need to obtain this from your previous Practice.
